Client Forms
In order to become a therapeutic horsemanship client at Wings of Hope you must have a diagnosed disability or challenge by a licensed medical healthcare professional.
All clients must complete the application process including a meet & greet evaluation at Wings of Hope and commit to a weekly session on the same day and time each week. Sessions are scheduled continuously September - May with camp and modified Summer opportunities in June & July.

If you are interested in becoming a client or you are a current client needing renewal paperwork please utilize these forms
to complete the paperwork and send the medical professional statement form to be completed by your physician.
